From this photo I took, you can tell the main focus is the flowers. The surroundings are all blurred out to create the Bokeh effect.
It's quite easy to do. You choose a main focal point, as I chose these flowers, and you focus onto them so the rest is blurred out. You don't want the un-blurred portion to be in the middle of the picture, so you put it to the side. I usually choose the left side because it looks better to me, but it doesn't matter.
